Case Study:
- Patient: 56-year-old male
- Chief Complaint: Chest discomfort and nausea
- Presentation: Mr. R is a 56-year-old male who presents to the emergency department with intermittent chest discomfort for the past 48 hours. He describes the discomfort as a burning sensation behind the sternum, worsened after meals and when lying flat. He has had several similar episodes over the last few months that resolved with over-the-counter antacids.
- Past Medical History:
- Hypertension
- GERD
- Hyperlipidemia
- Smoker (12 pack per day)
- No known cardiac history
- Vital Signs:
- BP: 138/84
- HR: 96
- RR: 18
- Temp: 98.4F
- SpO2: 97% on room air
- Initial Assessment and Plan by ED team: Given the patients description and history of GERD, the team treats him with a GI cocktail and discharges him with instructions to follow up with his PCP. No EKG or troponins were ordered.
- Outcome: Mr. R returns 24 hours later with worsening chest pain, shortness of breath, and diaphoresis. His EKG shows ST elevations in the anterior leads. Troponin is elevated, and cardiology is urgently consulted. He is diagnosed with an anterior STEMI.

- Write a one- to two-sentence problem representation summarizing the key clinical features with semantic qualifiers.
- List 46 relevant clinical features (signs and symptoms) from the case that should guide your differential diagnosis.
- Generate a prioritized list of 3-5 differential diagnoses based on this initial presentation. Support your selection of differentials with pertinent positives and negatives from the case.
- Choose your most likely diagnosis and provide a brief illness script (epidemiology, time course, key signs/symptoms, pathophysiology, treatment)
- Identify 23 cognitive biases present in the case. Name and describe how they influenced clinical judgment.
- Share one cognitive strategy or approach that could have prevented the error in this case.
- How has your nursing experience influenced the way you approach differential diagnosis and cognitive bias in this case?
- End your post by asking two open-ended questions related to this weeks course content to spark discussion with your peers. These should not be yes/no questions and must remain tied to concepts presented in Module 3.
