Effective Assessments
It is likely that your agency has a specific intake or assessment form that is standard for all clients. It is important to note, however, that assessment is a broad term and assessment tools come in a variety of shapes. An assessment may be a written biopsychosocial history where biological, psychological, and social factors (as well as spiritual and cultural) are explored.
